SCANNING ERRORS for Casstat and Teacher Evaluations


For the purposes of these instructions prompts to the user will be BOLD UNDERLINED and responses they should give will be in Italics after typing in the response always press the enter key.

Explanation of the Error Messages:

DO YOU WISH TO CONTINUE, POSITION OR HELP (Y, P OR H)?

  • Y – Yes
    Use this answer if you just wish to continue
  • P – Position
    Use this answer if you wish to check the position of the form. This is recommended if a form becomes jammed or you are uncertain if a form was read. If you press P and Press Enter you will receive the name on and number of the last form read correctly. Check the numbering and name, then select from the following options:
    1. CONTINUE SCANNING AFTER DISPLAYED FORM
    2. (If the form numbering is correct)

    3. BACK SPACE AND DISPLAY FORM
    4. (If you wish to go back one form and scan again)

    5. FORWARD SPACE AND DISPLAY FORM

      (If an error occurred but the form was scanned and you wish to continue with the next form)

      Before continuing make sure the forms in the right hand tray are in the correct starting position. Then Press Enter.

  • H – Help
    Use this if you wish to display the online error message

THE RESOLUTION OF A 5 (OR 10) RESPONSE FORM WAS REQUESTED, BUT THE FORM IN THE BOTTOM OUTPUT TRAY ON THE OPTICAL SCANNER ISN’T ONE.

Solution: The program will automatically take you back to Step 1. Check which forms you have and proceed from there.

INSERT MORE FORMS IN INPUT HOPPER

Three Cases:

  • Case 1 – The forms are not placed in the scanner properly.
    Solution: Make sure the forms are placed flat in the scanner and under the metalbar on the side nearest you.  When asked …(Y, P OR H)? Press Y
  • Case 2 - You pressed RETURN before placing the forms in the hopper.
    Solution:  Place your forms in the hopper now. When asked …(Y, P OR H)? Press Y
  • Case 3 – You didn’t place an End of Forms after your last form.
    Solution:   Place an End of Forms on the scanner now.   When asked …(Y, P OR H)? Press Y

KEY HAS BLANK ANSWERS (with a line of numbers indicating the blank answers)

  • If this is intentional
    IS THIS CORRECT? Y
  • Else
    IS THIS CORRECT? N
    Correct the answer key and start from the very beginning (Step 1)

JAMMED SCANNER

Solution:

  1. Gently pull the jammed forms directly towards the wall (away from you).
  2. …(Y, P OR H)? P

FORM REJECTED – TIMING MARK MISMATCH

Three Cases:

  1. The form was not recognized. This could be because the form was not aligned properly or it was upside down.  Solution:
    1. …(Y, P OR H)? P
    2. Go through the directions on using P on page 1
    3. Continue scanning
  2. A damaged form.  You will not be able to scan this form. 
    Solution:
    1. …(Y, P OR H)? P
    2. Go through the directions on using P on page 1
    3. Continue scanning
  3. The read head is dirty.
    Solution:
    1. Inform the lab assistant. They will help solve this problem.
    2. Once the scan head is clean, proceed.

Three Cases:

  1. The form was not recognized. This could be because the form was not aligned properly or it was upside down.  Solution:
